Custom Permission Slip for G-Scoutmate

Council: Sahuaro Girl Scout Council
Filename: permissionslip-sahuaro.rpt

To use, do the following:

1) Save this file into your G-Scoutmate folder (probably c:\program files\gscoutmate60).
2) Open G-Scoutmate and select "File" and "Preferences"
3) Click on the "SF" button next to Permission slip report file" and double-click on the permission slip file.
4) Click on the "Save" button.

If you want to revert back to the default permission slip, do the above steps, however you should double-click on the file named "permissionslip.rpt".

Additional Permission Slip Fields

Your council's permission slip has additional fields that need to be filled out.  When you're creating the permission slip, click on the second tab ("For Custom Permission Slips") and fill in the following fields (the corresponding field on your council's permission slip is also shown):

Text Field #1	Location Phone Number
Text Field #2	Location Cell Number
Text Field #3	Activities include
Text Field #4	Method of Transportation
Text Field #5	Leader Cell Number
